Animal Medical Center $500 Giveaway Terms and Conditions

By entering, all registrants agree to abide by these terms and conditions. Animal Medical Center (The Center) will make the final decisions. The Center has the sole discretion to change/alter prizes and/or rules and regulations accompanying prizes.

Eligibility:  

1. All registrants must be a minimum age of 18 to enter and to win. 
2. Should a registrant fail to complete the required documents and supply required information by the entry window deadline or submit with false information, the registrant will be disqualified.
3. All registrants must be residents of Kentucky to be eligible to enter the giveaway.
4. If you are an employee or staff member of Animal Medical Center of Frankfort, herein collectively referred to as “AMC” or the “Center,” an advertising or promotional agency of Animal Medical Center or any other enterprise that contributed promotional consideration (such as prizes, administration or hosting services), you are not eligible to enter the giveaway. This includes summer, part-time, full-time, and temporary staff members.  Employees’ Family members, including siblings, parents, significant others, children, grandchildren, or grandparents, are also not eligible to win. Also, people living in the same household or individuals otherwise excluded are not eligible to win.
5. If you are a Veterinarian or an employee or staff member of any veterinary Center, hospital, or treatment facility, people living in the same household or individuals otherwise excluded are not eligible to win, are not eligible to enter the giveaway. This includes summer, part-time, full-time, and temporary staff members. Family members of employees, including siblings, parents, significant others, children, grandchildren, or grandparents, are also not eligible to win. 

Entering:

1. Only one entry per household.
2. No purchase is necessary to enter or to win.
3. All registrants may enter the giveaway only once. Their names will remain in the drawing for the duration of the giveaway. Animal Medical Center will declare this time frame, which is subject to change at any time.
4. Registrants may enter the giveaway online through Animal Medical Center’s official entry page on our website. We will not accept entries through any other means. 
5. Animal Medical Center is not responsible for any technical difficulties entrants face, including difficulties sending/receiving emails or phone calls or accessing the Center’s website or social media accounts.

Redemption/Winning:

1. There is no cash redemption or any exchanges of prizes. The AMC account credit amount will be directly applied to the winners’ Animal Medical Center client account.
2. Only one winner per family/household.
3. Upon entry into the Animal Medical Center AMC account credit Giveaway, you will be entered to win One (1) Five Hundred Dollar ($500.00 USD) AMC account credit to Animal Medical Center of Frankfort, Kentucky. 
4. Any additional fees above and beyond the AMC account credit amount are the responsibility of the giveaway prize recipient.
5. Existing clients with outstanding account balances will have their balance deducted from the prize amount.
6. Registration for Animal Medical Center’s AMC account credit Giveaway is open from August 5, 2024 (8:00 a.m. EST) through September 30, 2024 (11:59 p.m. EST).
7. Two GIVEAWAY WINNERS will be selected randomly (using an online random selector). Prize winners will be announced October 9 during our ribbon cutting
8. Animal Medical Center will contact the winning registrants via email and phone and announce them on its Facebook and Instagram pages on or around October 9, 2024. If the winners do not respond within 48 hours, another registrant will be randomly selected.
9. The registrant must show valid photo identification at the time of prize redemption. If the registrant fails to show photo identification or documentation, Animal Medical Center may not release the giveaway.
10. The winner will be asked to create or update an account with Animal Medical Center and sign all necessary paperwork. Then, a $500 credit will be awarded to the winners’ Animal Medical Center client account. This includes a liability and photo release, which will release Animal Medical Center, all staff members, the owner, and any contest or giveaway sponsor from being held responsible.
11. The Center is not responsible for attempting to contact the winner more than once by phone and once by email. Should a winner not respond within 48 hours, then the Center holds no further obligation to that winner. Animal Medical Center may continue attempting to contact the winner/registrant. A different winner may be chosen should the Center not reach, obtain a response, or contact the original winner.
12. Small prizes besides the Grand Prize(s) may or may not be available. Prizes may have a deadline to be picked up. All prizes must be picked up or retrieved within 5 days of the original or attempted notification. This may be subject to change.
    1. If the winner does not claim a prize within the time deadline, the Center has the sole discretion to declare another winner.
    2. The prize becomes the Center’s property if it is picked up after the deadline. The Center reserves the right to pick a new winner or dispose of the prize in some other way.
13. The winner will not be allowed to pick up the prize until their eligibility to participate in the Giveaway has been confirmed and the required paperwork has been completed. Animal Medical Center may take steps to confirm eligibility but is not responsible for doing so.
14. The documentation (including but not limited to the liability release) must be completed and signed at the Center’s office before the winner is eligible to claim a prize. The winner must also bring a form of photo identification (showing the date of birth) and any other documents declared by the Center.
    1. Should the winner fail to bring photo identification, documentation, and proof of social security number, the Center will not release the prize to the winner.
    2. Animal Medical Center can declare another winner if the winner refuses to sign the required documents.
15. Prizes must be claimed in person. The AMC account credit amount will only be credited to the declared winner(s) Animal Medical Center account.
16. Those who win and claim prizes are held responsible for paying all taxes if applicable (including but not limited to federal, state, county, and local taxes). Taxes will be based on prize value.
    1. The Center may extend the expiration date of any or all prizes.
    2. Animal Medical Center is also not responsible for reimbursing or replacing any prize.
17. The Center has the sole discretion to change/alter prizes and/or rules and regulations accompanying prizes.
18. The winner authorizes Animal Medical Center and all its affiliates, successors, and assignees permission to use their pictorial, likeness, name, or video and/or sound recordings for marketing, promotion, broadcast, print, web, or otherwise.
19. Animal Medical Center will make the final decisions. By entering, all registrants agree to abide by these terms and conditions. 
20. Animal Medical Center has no responsibility for losses, injuries, or damages caused by accepting these terms and conditions and entering into programs, contests, memberships, and giveaways. Terms and agreements are subject to change. Should they change, registrants/members still agree to alterations. The registrant/member is responsible for regularly checking the terms and agreements to ensure they are aware of any modifications and still agree to follow such rules and regulations. The Center reserves the right to delete, alter, or add any terms, conditions, and rules.
21. Animal Medical Center shall not be held responsible or liable should there be a printing, publishing, transmission, communication error, or other technical error related to the contest, campaign, program, or giveaway, including but not limited to the type or number of prizes.
22. Animal Medical Center has the right to cancel or abandon the contest, campaign, program, or giveaway at any time or to do anything it chooses within the bounds of the law to remedy a given error, problem, or situation or discontinue it.
23. Animal Medical Center’s maximum liability for any giveaway, campaign, or contest shall be the lesser of the prize(s) value or Five Hundred Dollars ($500.00).
24. Registrant and/or winner shall indemnify and defend Animal Medical Center and its managers, officers, directors, employees, agents, successors, and assigns against any and all losses, damages, liabilities, deficiencies, claims, actions, judgments, settlements, interests, awards, penalties, fines, costs, expenses of whatever kind, including professional and attorney fees arising out of any claim alleging any breach or non-fulfillment of any representation warranty or covenant, negligence, omission, injury, bodily injury, death, reckless or willful misconduct, or failure to comply with any law, rule, regulation or code.
